What is the output if you invoke erlang directly than attempt to load couchdb?

erl -init_debug -couch_ini

application:load(crypto), application:start(crypto).
application:load(couch), application:start(couch).

We had tried building 1.2.0 from source code ourselves and received the exact same error. We have also tried a few suggested fixes involving setting the ERL_LIBS variable we found in various places, but none worked. At this point I suggest you submit an official bug report to the couchdb developer community. Or build the older 1.1 version for now.
