I'm running Nginx compiled with Pagespeed, PHP-FPM, and APC to serve a forum. PHP-FPM and Nginx communicate via UNIX socket. All request to the server are coming from 127.0.0.1 according to the forum software and access.log.

Nginx is compiled with the RealIP module and set_real_ip_from is set in nginx.conf for 127.0.0.1 and unix:.

Is this a problem with the front-end Nginx proxy?

It sounds like you're not using the correct header to set the real IP. You should be using real_ip_header X-Forwarded-For; in your config.
