Im aware that there are front-end/proxy nginx servers which handle incoming connections, including requests to custom apps.

Im running a server as a custom app and all request are been logged as if it originated from 127.0.0.1

If I open the port of the custom app and use it in the request then the correct public IP is logged by my custom server.

Any workaround of been able the get original IP from request without opening a port?

You would look for the X-Forwarded-For header, this will have the real IP.
